T-Card System Regulation

Description:

T-cards system should be used for personnel tracking and record purpose, which enter to process area, to enable emergency response management, Evacuation and rescue process as well as headcount of evacuees when required. Without T-Card, entering to process area is prohibited. For every single access/egress to process area, the T-Card shall be processed by check point regardless the numerous times of incoming/ outgoing of taskforce.

General Regulation:

1. The locations of check points to submit T-Card are at main gates of process area.
2. Authorized staff (s) must assign for carry of T-Card system as check point officer.
3. Special TBM for each crew (Due to mentioned activities in PTW) must conduct by HSSE team before starting the T-Card process.
4. The T-cards will be submitted to those personnel who have successfully passed H2S, Emergency response and evacuation training courses as well as defensive driving for drivers.
5. Each T-Card carries a unique digit/Number (from 001 to 110% of project staff amount).
6. Each area must using different color of T-Card.
7. Upon submitting of special gate pass, H2S cards and forbidden objects to check point by the person that enters process area, He/ She shall explain to check point the reason of his entrance and his destination. It includes registration of PTW in check point log book.
8. After the special gate pass, H2S card, Phone, Cigarette and lighter collected and T-Card submitted by check point, Special protective equipment will be submitted to entered person.
9. The T-Card shall be attached to the shirt or working cloths of staff using attached clips.
10. The T-Card shall be kept with the person as long as he is inside the working area. It cannot be borrowed from or lend to others.
11. The T-Card shall be retued back to check points when the person egresses from working area and personal gate pass, H2S card, Phone, Cigarette and lighter will be retued back to the person. Special protective equipment shall be retued back to check points at the same time.
12. In case of emergency evacuation, the T-Card shall be kept with each person at muster points for headcount purpose (No need to change the T-Card when escape from process area to muster points).

HSSE staff is responsible for monitoring and check the staff T-Cards in working areas during working time.
